Who We Serve
- You must have Medicaid or be eligible for Medicaid and be eligible for Home Care Services as determined by the New York State Independent Assessor Program (NYIAP).

Do I need to have Medicaid to be eligible for CDPAP services?
Yes, you must be Medicaid eligible and be eligible for home care services as determined by your doctor.
How do I start the process for CDPAP if I am 18 years of age or older?
Visit the NYS Independent Assessor (NYIA) website to schedule an assessment or call 1-855-222-8350.
How do I start the process for CDPAP for a child under the age of 18?
Contact the child's doctor to determine if the child qualifies for services.
Who can be my worker?
It’s your choice! It can be a friend, family member or neighbor. However, workers can not be a spouse, designated representative, or parent of a child younger than 21.
Does my worker need to have certification?
No. Workers do not need to be certified, you will be responsible to train the workers.
