Community Transition Assistance Program
RCIL’s Community Transition Assistance Program offers personalized support to help individuals navigate systems and secure benefits. Together, we can work towards a future where every individual has the resources and support they need to thrive in their community.
RCIL’s disability rights advocates provide services tailored to each person’s unique goals, making sure that individuals’ rights are not compromised. We assist individuals who face challenges navigating services, systems, and the community in areas like public benefits, housing applications, home modifications, and independent living skills training, with specialized advocates focusing on systems such as education and employment.
This program is supported by Herkimer County Office For The Aging.
Program Features
Advocates offer personalized services to promote independent community living.
- Assistance with public benefits applications, including SNAP, Medicaid, and Social Security.
- Support with housing applications and access to home modifications.
- Independent Living Skills Training (ILST) for tasks like shopping and paying bills.
- Guidance on navigating services, systems, and community resources.
- Advocacy to protect individual rights and promote independence.
- Specialized advocacy in education and employment systems.
- Information and Referral (I&R) services for resources and information.
Who We Serve
- Advocates work with anyone with any type of disability, including a disability perceived by the person requesting the service. There is no need to provide documentation of any medical condition or disability.
