Want to live in your own home or apartment? It's your choice! The Open Doors / Money Follows the Person and the Olmstead Housing Subsidy programs provide transition specialists and housing specialist to help people move from long-term care facilities back into their homes and communities.
The Open Doors Program and the Olmstead Housing Subsidy Program help people move from nursing homes or intermediate care facilities to homes in the community. These programs help you find housing, services, and supports you need to live independently in the community.
Program Features
The Transition Specialist and the Housing Specialist help people get the care and services they need at home and in their community as well as assist participants in finding affordable, accessible housing.
- Finding Affordable and accessible housing.
- Identify services you need in the community.
- Information and skills you’ll need once you return home such as budget planning how to grocery shop, and setting independent living goals.
- Connecting you with service coordination or case management.
- Setting up aid services either traditionally or through CDPAP.
Who We Serve
- Must be living in a nursing home for more than 60 consecutive days.
- Wants to live independently in the community and can do so safely.
- Are moving to a qualified community residence such as: owned or rented home, residence in the community with no more than four unrelated individuals residing at, rented apartment with an individual lease.
- Are a Medicaid recipient.

RCIL can help by teaming you with a Transition Specialist and a Housing Specialist who will partner with you to establish what needs you have in order to live safely in the community.
I don’t have a cell phone anymore. How will I communicate with my specialist?
The Open Doors Program can help by providing participants with cell phones at no charge to them.
I do not know what services are available to me in the community.
Transition Specialist and Housing Specialist are knowledgeable on what services are available to you in your community. They can help you gain access to these services.
I need someone to talk to that can relate to my situation. Can the program provide that?
Yes, the program can connect you with a Peer Support Specialist that has similar experiences and can relate to what you are going through.
I can’t afford a security deposit, furniture, or items I need to be comfortable in my own home, what do I do?
Your assigned Housing Specialist can submit request to have the program help fund a security deposit and basic furnishings to start you with in your new home.
